Food help for stroke recovery
Each year, ~795,000 Americans have a stroke, and ~7 million live with after-effects. Food is central to recovery: 30-65% of survivors have dysphagia (swallowing difficulty), and the right diet can reduce second-stroke risk by up to 50%. This page covers dysphagia, MIND/DASH diets, and benefits.
Post-stroke dysphagia
Aspirating food or liquid into the lungs is a leading cause of pneumonia and death post-stroke. A speech-language pathologist (SLP) evaluation is essential:
- Modified Barium Swallow Study (MBSS / videofluoroscopy) — gold standard. Covered by Medicare/Medicaid CPT 92611.
- FEES (Fiberoptic Endoscopic Evaluation of Swallowing) — nasal-camera alternative.
- The SLP will recommend a specific IDDSI level and techniques (chin-tuck, double swallow, alternate liquid/solid).
IDDSI levels (International Dysphagia Diet Standardisation Initiative)
2019 global standard for modified textures. Your SLP indicates your level:
- Liquids:
- Level 0 — thin: water, clear juice — permitted without dysphagia.
- Level 1 — slightly thick.
- Level 2 — mildly thick (nectar consistency) — pours slowly.
- Level 3 — moderately thick (honey) — spoon barely slides.
- Level 4 — extremely thick (pudding) — holds shape.
- Solids:
- Level 4 — pureed (stage-1 baby food consistency).
- Level 5 — minced and moist (~4mm pieces).
- Level 6 — soft and bite-sized (~1.5cm).
- Level 7 — regular (everything) or regular easy-to-chew.
Thickeners and commercial products
- Thick-It — thickening powder (modified starch or xanthan gum). $15-25 container.
- SimplyThick (xanthan gum) — commercial gel. Covered by Medicaid in some states with SLP prescription.
- Thicken Up Clear (Nestlé) — xanthan gum. Dissolves without lumps.

- Covered by Medicare Part B with prescription as a medical supply.
MIND diet (dementia + secondary stroke prevention)
Combines Mediterranean + DASH. 53% dementia reduction with high adherence (Rush U):
- Leafy greens — 6+/week.
- Other vegetables — 1+/day.
- Berries — 2+/week (blueberries especially).
- Nuts — 5+/week.
- Olive oil as primary fat.
- Whole grains — 3+/day.
- Fish — 1+/week.
- Beans — 3+/week.
- Poultry — 2+/week.
- Wine — 1/day (only if you already drink; 0 if not).
- Limit: butter/margarine (<1 tbsp/day), cheese (<1/wk), red meat (<4/wk), fried food (<1/wk), sweets (<5/wk).
DASH diet (blood pressure)
Reduces systolic BP ~11 mmHg. Each 10 mmHg reduction = 27% fewer recurrent strokes:
- Sodium <2,300 mg/day (ideally <1,500 per AHA).
- Potassium 3,500-4,700 mg/day: bananas, potatoes, spinach, beans, oranges.
- Magnesium 320-420 mg/day: almonds, spinach, cashews.
- Calcium 1,000-1,200 mg/day: low-fat dairy, broccoli, kale.
- BP control <130/80 with medication if diet alone insufficient.

SNAP, Medicare, Medicaid
- SNAP covers all MIND/DASH foods — vegetables, berries, nuts, olive oil, fish, whole grains.
- Medicare covers: post-stroke rehabilitation (PT, OT, ST), SLP evaluation, MBSS, durable medical equipment, post-acute meals.
- Medicare Advantage 2026: supplemental benefits may include tailored meals, transportation to appointments, thickeners.
- Medicaid (expansion states): up to 138% FPL (~$21,597 individual 2026).
- Senior/disabled SNAP medical deduction: for expenses >$35/month (thickeners, copays).
