38.4 million Americans (11.6%) live with diabetes — 2M Type 1, 36M Type 2. Another 97.6 million adults (38%) have prediabetes (CDC 2024). Diabetes costs $19,736/year/person in medical expenses. Hispanic/Latino (12.5%), Black (12.1%), and American Indian/Alaska Native (13.6%) communities have 50–80% higher prevalence than non-Hispanic white (7.4%).

Medicare programs for diabetes
- Medicare Diabetes Prevention Program (MDPP) — free group classes for Medicare beneficiaries with prediabetes. Covers 22 sessions over 12 months under CMS HCPCS G9873-G9890. 5% weight loss target. Enroll: medicare.gov/coverage/diabetes-prevention-program.
- Medical Nutrition Therapy (MNT) — Medicare Part B covers 3 hours/year first year + 2 hours/year subsequent with registered dietitian under CPT 97802-97804. Diabetes and kidney disease qualify.
- DSMES (Diabetes Self-Management Education and Support) — 10 hours initial + 2 hours/year Medicare. Codes G0108/G0109. Find accredited program: diabeteseducator.org.
- Continuous glucose monitors (CGM) — Dexcom G7, FreeStyle Libre 3 covered by Medicare since 2023 if you use insulin or have documented hypoglycemia. No more multiple finger-sticks per day.
- $35 insulin cap (Inflation Reduction Act, PL 117-169) — since Jan 1, 2023, Medicare Part D and Part B insulin costs max $35/month/prescription. Applies to Lantus, Humalog, NovoLog, Tresiba, Toujeo, Basaglar.
Tribal programs — Special Diabetes Program for Indians (SDPI)
Under PL 105-33 (Balanced Budget Act 1997, as amended), the Indian Health Service administers $160M/year in grants to 302 SDPI programs at tribal, IHS, and urban Indian health centers. SNAP for diabetes-friendly groceries
SNAP covers all diabetes-friendly foods. Strategic shopping:
- Slow carbs: steel-cut oats, barley, quinoa, black/pinto/garbanzo beans, lentils, brown rice.
- Non-starchy vegetables: broccoli, spinach, kale, cauliflower, zucchini, tomatoes, peppers.
- Lean proteins: skinless chicken, fish (canned tuna in water, salmon), eggs, tofu, beans.
- Healthy fats: avocado, olive oil, nuts (almonds, walnuts), chia/flax seeds.
- Low-glycemic fruits: berries, cherries, apples, pears, oranges, kiwi (moderate portions).
- Dairy: unsweetened Greek yogurt, low-fat cheese, skim or unsweetened non-dairy milk.
SNAP medical deduction (7 CFR 273.9(d)(3)) — if you're 60+ or receive SSI/SSDI for disability, out-of-pocket medical expenses >$35/month (includes insulin, test strips, lancets, copays, transport to appointments) reduce your counted income and increase your SNAP benefit. Bring receipts to your SNAP appointment.
SSI/SSDI for diabetes with complications
Diabetes alone rarely qualifies. Qualifying complications:

- VA Diabetes Self-Management Program — eligible veterans receive MDPP, dietitian, CGM, insulin with no copay in many cases.
