Food help for chronic pain
~52 million US adults (20.9%) have chronic pain (CDC NHIS 2023), and ~17 million have high-impact chronic pain (HICP) that limits daily activities. Inflammation, vitamin D deficit, gut microbiome alterations, and omega-3 deficiencies amplify pain. Diet is a real lever, not a placebo. This page covers food + opioid alternatives + federal/state programs.
Anti-inflammatory diet (level-A evidence for multiple conditions)
- Fatty fish 2-3x/week: salmon, sardines, mackerel, anchovies, trout. Omega-3 EPA/DHA reduces inflammatory prostaglandins.
- Dark leafy greens: spinach, kale, chard, arugula. Antioxidants + magnesium.
- Berries and colorful fruits: blueberries, strawberries, raspberries, cherries (especially tart cherry — reduces joint pain in studies).
- Extra virgin olive oil: oleocanthal works like natural ibuprofen.
- Nuts and seeds: walnuts, almonds, chia seeds, flax seeds, pumpkin seeds.
- Anti-inflammatory spices: turmeric + black pepper (curcumin), ginger, cinnamon, rosemary.
- Dark cocoa >70%: flavonoids reduce inflammation.
- Green tea: EGCG — 2-3 cups/day.
PRO-inflammatory foods (limit or avoid)
- Refined sugar: causes major inflammation spike. AHA recommends <25g/day (women), <36g (men).
- Industrial seed oils: soy, corn, canola, sunflower — high omega-6 (current omega-6:omega-3 ratio >15:1 vs ideal <4:1).
- Trans fats: margarines, fried foods, industrial baked goods.
- Processed meat: bacon, sausage, deli meats, hot dogs — AGEs (advanced glycation end products).
- Refined carbs: white bread, white rice, white pasta, soda.
- Excess alcohol.
- Some nightshades (subgroup): tomato, potato, eggplant, bell pepper — ~30% of people with arthritis worsen with these. Try 4-week elimination.
Supplements with evidence for pain
- Omega-3 (EPA + DHA) 2-4 g/day: Cochrane review 2017: equivalent to NSAID in RA, without GI side effects.
- Vitamin D3 2,000-5,000 IU/day: deficiency (<30 ng/mL) is strongly associated with chronic pain. Ask for 25-OH-D test.
- Magnesium 300-500 mg/day: magnesium glycinate or citrate. Reduces neuropathic pain, migraine, fibromyalgia.
- Turmeric/curcumin 500-1,500 mg/day with piperine: Cochrane: equivalent to celecoxib in OA, no GI bleed.
- Glucosamine + chondroitin: mixed evidence, but ~1/3 respond in OA.
- SAMe 400-1,200 mg/day: evidence for OA and depression.
- Probiotics: microbiome altered in fibromyalgia, IBS, chronic pain. Lactobacillus + Bifidobacterium.
- Vitamin B12: deficiency causes neuropathy. Vegans, vegetarians, >50, metformin, PPI users — high risk.
Medical cannabis (38 states + DC as of 2026)
Medical cannabis is legal for chronic pain in 38 states + DC + Puerto Rico + USVI + Guam:
- States without programs: ID, KS, NC, SC, TN, WI, WY (as of 2026; changes rapidly).
- Cost: ~$100-300 annual state registration + ~$50-300/month product. NOT covered by Medicare/Medicaid/commercial insurance — remains federal Schedule I.
- Products: flower, vape, edibles, tinctures, topicals. THC + CBD in various ratios.
- Discount programs: veterans, disability, low-income — vary by state.
- CBD (no THC) federally legal — less effective for severe pain, but no high. ~$30-100/month.
- Risks: dependence (~10%), drug interactions (warfarin, anticonvulsants), pregnancy NO, under 25 watch brain development.
Non-opioid alternatives (HHS Pain Management Best Practices 2019)
- Physical: physical therapy, aerobic exercise, yoga, tai chi, aqua therapy. Covered by Medicare/Medicaid.
- Manual: chiropractic, massage, osteopathy. Medicare covers chiropractic for subluxation; much else does not.
- Mind-body: CBT-CP (cognitive behavioral therapy for pain), MBSR (Mindfulness-Based Stress Reduction), biofeedback. Increasingly covered.
- Acupuncture: Medicare covers 12-20 sessions/year for low back pain (2020+). Some Medicaid programs too.
- Non-opioid pharmacology:
- Acetaminophen — OTC, up to 3 g/day.
- NSAIDs — ibuprofen, naproxen, celecoxib (Celebrex).
- Topical NSAIDs: diclofenac gel (Voltaren OTC), Pennsaid.
- Antidepressants: duloxetine (Cymbalta), amitriptyline, nortriptyline — first-line for fibromyalgia, diabetic neuropathy.
- Anticonvulsants: gabapentin (Neurontin), pregabalin (Lyrica) — neuropathy.
- Injections: epidural steroids, nerve blocks, RFA radiofrequency.
- Stimulation: TENS, spinal cord stimulator, Nevro HFX devices.
- Ketamine infusion: growing evidence for refractory neuropathic pain.
Federal benefits for chronic pain
- SSDI/SSI: chronic pain alone does NOT qualify, but causing conditions (fibromyalgia, lupus, arthritis, spinal injury, diabetic neuropathy) do. Document with objective tests.
- Medicare: eligibility via SSDI after 24 months (no need to wait until 65).
- Medicaid: up to 138% FPL (~$21,597 individual 2026) in expansion states.
- FMLA: 12 unpaid weeks/year, job + insurance protected.
- ADA + Section 504: reasonable accommodations at work (ergonomic chair, flexible schedule, remote work).
- SNAP medical deduction: 60+ or disability, expenses >$35/month (PT, supplements, copays, transport) deductible from income.
- EITC and Child Tax Credit if you can do some work.
