Food help for breast cancer patients and survivors
290,000 new diagnoses of invasive breast cancer per year in women + 2,800 in men (American Cancer Society 2026). 4.1+ million survivors in US. Black women have 40% higher mortality despite lower incidence (delayed care, more aggressive subtypes like triple-negative). Diet is a powerful modifiable factor: Mediterranean diet reduces recurrence 25-50%; alcohol increases risk 7-12% per drink/day.
Mediterranean diet and recurrence reduction
- CORDIOPREV/PREDIMED study: women on Mediterranean diet + extra virgin olive oil had 68% less breast cancer vs. low-fat diet.
- Increase:
- Cruciferous vegetables (broccoli, cauliflower, kale) — sulforaphane and indole-3-carbinol modulate estrogen.
- Fruits (especially berries, pomegranate).
- Fatty fish 2x/week — salmon, sardines.
- Nuts, seeds (flax, chia).
- Extra virgin olive oil.
- Beans, lentils.
- Whole grains.
- Green tea — EGCG inhibits tumor growth in studies.
- Soy — NO LONGER A TABOO: 2009-2020 studies confirm whole soy (tofu, tempeh, edamame) is SAFE and possibly beneficial even for ER+ cancer. Soy isoflavones are selective estrogen receptor modulators (natural SERMs). 1-2 servings/day. Avoid ONLY isolated isoflavone supplements.
- Reduce: red meat (<3 servings/week), processed meats, ultra-processed foods, added sugar, refined flours.
- Limit whole-fat dairy — studies suggest association with ER+ cancer. Skim or non-dairy.
Alcohol and breast cancer
Any amount of alcohol increases breast cancer risk:
- 1 drink/day — +7% risk.
- 2-3 drinks/day — +20% risk.
- Alcohol increases circulating estrogen, damages DNA, interferes with folate.
- Recommendation: survivors should avoid alcohol entirely or limit to <3 drinks/week.
Vitamins and supplements
- Vitamin D 1,000-4,000 IU/day — common deficiency; studies suggest lower recurrence with levels >30 ng/mL. Check levels 1-2x/year.
- Omega-3 EPA/DHA 1-2 g/day — reduces inflammation.
- Calcium 1,000-1,200 mg/day — important with AI (induces osteoporosis).
- Vitamin C, E moderate — NO megadoses during chemo (interfere).
- AVOID: high-dose antioxidants during chemotherapy, hormonal supplements (DHEA, high-dose melatonin), St. John's Wort (interferes with tamoxifen), ephedra, kava.
- Turmeric: consult oncologist; modulates CYP3A4 which metabolizes tamoxifen.
Managing treatment side effects
- Nausea (chemo): fresh ginger/capsules (Sea-Band), small frequent meals, BRAT diet, cold smoothies.
- Mucositis (Adriamycin, etc.): Magic Mouthwash (pharmacy mix), baking soda + salt in water, ice cream, popsicles, avoid acidic/spicy/dry.
- Weight loss: Ensure Plus, Boost Plus, protein smoothies. Covered by SNAP.
- Weight gain (with AI or chemo): 50-96% of patients gain weight. Mediterranean diet, exercise.
- Hot flashes (with tamoxifen/AI): avoid spicy foods, alcohol; soy isoflavones may help; gabapentin prescribed.
- Constipation (opioids): water 64+ oz, fiber, prunes, MiraLAX, Senokot.
- Diarrhea (Verzenio, Tukysa): Imodium after 4 stools, BRAT diet, electrolyte hydration.
Endocrine treatments (ER+/PR+ cancer)
- Tamoxifen — pre-menopausal. Generic $5-15/month. 5-10 years.
- Aromatase inhibitors (AI):
- Anastrozole (Arimidex) — generic $5-15/month.
- Letrozole (Femara) — generic $5-15/month.
- Exemestane (Aromasin) — generic.
- CDK4/6 inhibitors (advanced ER+ cancer):
- Palbociclib (Ibrance) — ~$13,000/month list. Pfizer Oncology Together program: $15/month with private insurance, free <500% FPL uninsured.
- Ribociclib (Kisqali) — ~$13,000/month. Novartis Patient Assistance.
- Abemaciclib (Verzenio) — ~$15,000/month. Lilly Cares Foundation.
- Oral SERDs:
- Elacestrant (Orserdu) — FDA approved 2023 for ESR1+. Stemline Patient Support.
- IRA Medicare Part D $2,000/year cap (PL 117-169) — critical for CDK4/6.
HER2+ treatments
- Trastuzumab (Herceptin) — biosimilars Ogivri, Herzuma, Kanjinti, Trazimera, Ontruzant 30-50% cheaper.
- Pertuzumab (Perjeta) — combined with Herceptin for early tumors.
- T-DM1 (Kadcyla) — antibody-drug conjugate for residual HER2+.

SSI/SSDI under Listing 13.10 (Breast cancer)
- Listing 13.10A — breast cancer with distant metastases (MO Stage IV).
- Listing 13.10B — inflammatory breast cancer (aggressive form with skin changes).
- Listing 13.10C — recurrence post-surgery/radiation/chemo.
