Eligibility
Medicare-certified hospice care provider. Comprehensive end-of-life care for terminally-ill patients (typically prognosis ≤6 months). Includes nursing, medication, equipment, counseling, social work, and bereavement support. Most accept Medicare and Medicaid; many sliding-scale for the uninsured.
